PKHeX doesn't check validity of TID/SID. A combination of 0 and 0 can be rolled naturally, just unlikely. 2. The OT is valid, since it is a possible option to start for the game. Granted, HOME is suspicious when matched with TID and SID of 0, but nothing out of the bounds of legal. 3. While PID 0 is unlikely, it is still a possible roll. Tho this PID certainly looks hacked, when you take the OT/TID/SID into account. There's no rule that says a hacked Pokémon can't be legal and fishy looking at the same time. The same basis of calculation TID/SID xor against PID applies. (same formula as SWSH) The only exception is that if the mon has a fateful encounter flag, the game forces it to be displayed as Square Shiny, even if the PID is Star shiny.
