1. PKHeX doesn't check validity of TID/SID. A combination of 0 and 0 can be rolled naturally, just unlikely.
2. The OT is valid, since it is a possible option to start for the game. Granted, HOME is suspicious when matched with TID and SID of 0, but nothing out of the bounds of legal.
3. While PID 0 is unlikely, it is still a possible roll. Tho this PID certainly looks hacked, when you take the OT/TID/SID into account.

There's no rule that says a hacked Pokémon can't be legal and fishy looking at the same time.

There's no restriction. A shiny when transferred to SWSH, can be SQUARE or STAR shiny.
The same basis of calculation TID/SID xor against PID applies. (same formula as SWSH)

The only exception is that if the mon has a fateful encounter flag, the game forces it to be displayed as Square Shiny,
even if the PID is Star shiny.

This is the part that answers your question. Most shinys transferred from Bank will be star shiny, but there's a small chance they will become square shiny instead.
