IIRC, the same Pokémon GO avatar but without the Mimikyu hat, and the Pichu didn't have that witch hat either.
Before tht, hmm...wasn't it the same avatar with a Magikarp hat (and without the Pichu)?

Close! Previously it was Avatar in Red jacket and Magikarp Hat, with Pichu w/ Original Ash Cap

 

Also, I think yours has been that image this whole time, as far as I'm aware ><

Mine's a bit tricky I guess. I changed it recently, when I changed my in-game outfit, but the only visible difference is the top's color (which is barely visible due to the small portion that's shown and the purple on the newer pic being almost black anyway)
